The short answer: Write down when every US policy ends and where it will actually pay. Do not treat COBRA, a Marketplace plan, Medicare or a travel policy as international family health insurance until the insurer confirms overseas treatment, routine care, evacuation and claims in writing.
Confirm the last day of US cover
Ask the employer or insurer for the exact termination time, dependent treatment, claims run-out and whether care overseas is covered before that date. Coverage can end on the last day worked, the end of the month or another plan-defined date.
COBRA may allow temporary continuation of group coverage after a qualifying event, but it preserves the existing plan rather than turning it into worldwide insurance. Network, overseas emergency and direct-billing rules still come from that plan. Compare cost and usefulness against a dedicated transition policy.
- End date and time for each family member
- Overseas emergency and routine-care wording
- Deductible and out-of-pocket balances already met
- Open claims, prior authorisations and planned treatment
- COBRA election, payment and retroactive-cover deadlines
Understand what Medicare does not follow overseas
Original Medicare usually does not cover healthcare outside the United States, apart from limited situations involving a nearer foreign hospital or certain travel through Canada. In most overseas cases the person pays all costs. Medicare drug plans do not cover prescriptions bought outside the United States.
Moving abroad does not necessarily make future Medicare enrollment decisions disappear. Part B late-enrollment penalties and special-enrollment rights can depend on current employment and group coverage, not simply foreign insurance. People approaching 65 or leaving active employer coverage should get individual guidance from Medicare or Social Security before dropping or delaying a part.
Worth knowing: Medigap and Medicare Advantage policies can have limited travel benefits, but plan wording, time limits and service areas matter. They are not a substitute for checking long-term overseas cover.
Choose transition and long-term cover for different jobs
Travel medical insurance can help with a defined trip or initial period but may exclude routine care, pre-existing conditions, maternity, chronic treatment or long residence. International health insurance is designed for broader ongoing care, but underwriting, geography, provider networks, deductibles and evacuation rules vary.
Map where care will happen: local outpatient clinics, suitable paediatric and emergency hospitals, regional centres such as Singapore, and any planned treatment in the United States. Compare the total family exposure, not only the premium.
- Annual and lifetime limits, deductible and co-pay
- Pre-existing-condition decision for each person
- Outpatient, mental health, therapies and dental
- Maternity, newborn and congenital-condition rules
- Direct billing, guarantee letters and claims currency
- Evacuation destination, companion parent and return travel
Build a clinical handover before records become hard to reach
Request a concise clinical summary for each family member: diagnoses, allergies, medication names and doses, surgeries, immunisations, investigations, treatment plans and relevant family history. Add specialist letters, imaging reports, dental and optical records where useful.
Plan medicines by active ingredient and destination legality. A US prescription is not automatically valid abroad, controlled medicines may need advance permission and brand names differ. Carry original packaging, a clinician letter and enough lawful supply for the transition without stockpiling blindly.
Keep a route back into US care if it matters
If the family expects specialist follow-up, college-age dependants, regular US visits or future Medicare use, model the cost of keeping appropriate domestic cover or paying cash. Confirm whether an international plan includes elective care in the United States; US-inclusive cover is often priced differently.
Save insurer emergency numbers, policy documents, member IDs, exclusions and the claims process offline. Make sure both adults can access them and know which hospital to use during the first 72 hours.
What each cover type is actually for
Policy wording controls. Use this table to ask better questions, not to assume coverage.
| Cover | Useful role | Main check before relying on it |
|---|---|---|
| Employer plan | Care while employment coverage remains active | Overseas network, emergency wording and end date |
| COBRA | Temporary continuation of the same group plan | Premium, election timing and overseas usefulness |
| Marketplace plan | US individual medical cover within its service arrangements | Residence, network, move reporting and overseas limits |
| Medicare | Eligible care mainly within the US | Limited foreign exceptions and enrollment consequences |
| Travel medical | Short transition or defined trip risk | Duration, residence, routine care and exclusions |
| International health | Ongoing care across named countries or regions | Underwriting, limits, hospitals, evacuation and renewal |
